Subgrantees Overview

 

Our subgrantees are comprised of non-profit organizations that provide a multitude of services to ultimately improve not only healthcare, but quality of life for the people of Baton Rouge. These organizations will utilize grants with the primary goal of expansion which will improve access to services as well as the capacity to serve more of those seeking care.

EXCELTh

EXCELTh, inc is a private, not-for-profit organization founding in 1991. It is funded as a Community Health Center and is designated as a Federally Qualified Health Center. It aims to provide excellence in community-based health care that increases access, reduces health disparities, and improves health outcomes. Presently, the EXCELTh Primary Care Network includes 3 fixed health centers, a family dental center serving Orleans Parish, and a behavioral health program serving East Baton Rouge Parish. It provides comprehensive services ranging from pediatrics to geriatric care, as well as, care coordination, social services, and pharmacy services.

Open Health/HAART

HIV/AIDS Alliance for Region Two (HAART) is a 501(c)(3) non-profit community based organization with a rich history of providing services to the underserved citizens of the Baton Rouge area. Continuum of care services includes early intervention services, housing assistance, medical case management, mental health counseling, and more.

Primary care providers for healthy Feliciana

PCPFHF is a 501(c)(3) not-for-profit organization that has served the residents of East Feliciana Parish since 1999 and West Baton Rouge Parish since 2008 providing quality, affordable health care regardless of ability to pay. It is comprised of 5 stand alone clinics in East Feliciana, 4 of which are adjacent to each of the public schools within the East Feliciana Parish Public School System. PCPFHF offers primary care, dental care and behavioral health care. It currently provides behavioral health services in 34 Livingston Parish Schools and 11 Tangipahoa Parish Schools.

CareSouth

CareSouth is a private non-profit Federally Qualified Health Center that provides a comprehensive range of community-based medical, dental, and behavioral health services. Their health center is guided y consumer-majority volunteer board of directors whose mission is to provide comprehensive, high quality health care with compassion and respect for all. It provides a broad range of medical and dental services in Baton Rouge, as well as, satellite locations in Donaldsonville and Plaquemine.
